Petition for Late Withdrawal

After the 12th week, a student who was the victim of extenuating circumstances and did not qualify for a Medical Leave of Absence may petition for a Late Withdrawal from the University.

You must review the circumstances for which a request may be approved or denied before completing the petition:  Late Withdrawal due to Extenuating Circumstances.

Use this form to request a Late Withdrawal for individual courses or from all courses for a given term.

Petitions submitted for a course or courses prior to final grades being assigned, will automatically be denied.

NOTE: The deadline for filing a Petition for Late Withdrawal under extenuating circumstances is the last day of the full semester following the term from which the student is seeking withdrawal.  

The Office of the Registrar will only send student record information to your secure EKU email address. 

If yes, you are requesting a “W” in each and every class you were/are enrolled in, in the term indicated above.

List each class you are requesting a withdrawal from.  Include CRN, Class Prefix (e.g. BIO), Class Number (e.g. 101), and Instructor Name.
